Service Needs Assessment survey by NYC Department for the Aging

The NYC Department for the Aging (NYC Aging) is deeply committed to helping the approximately 1.8 million older New Yorkers age in their homes, as well as in creating a community-care approach that reflects a model age-inclusive city.

As the number of older New Yorkers 60+ continues to grow, the valuable information collected will be used to better serve and inform programming and advocacy. In an effort to better identify the needs of all older New Yorkers and their caregivers, and to ensure the provision of safe and inclusive services for diverse older adults, NYC Aging have put together a comprehensive service needs assessment.

If you have a family/friend/colleague who are over 60 or are caregivers, encourage them to complete the survey. The surveys are to be completed by:

  • Individuals aged 60 years or older; or
  • Caregivers of individuals aged 60 years or older

This survey will take about 20 minutes to complete. At the end of the survey, individuals will be invited to provide their contact information if you would like to enter a drawing for a giveaway for a $50 gift card sponsored by the Dept of Aging. Fifty winners will be selected.

The survey is offered:

  1. Online on.nyc.gov/agingsurvey or www.nyc.gov/aging.
  2. Hardcopy (contact the Dept of Aging directly)
  3. In person at community events.
  4. Scanning the QR Code.

The survey is available in the following 11 languages: Arabic, Bengali, Chinese, English, French, Haitian Creole, Korean, Polish, Russian, Spanish, and Urdu. All responses to this survey are anonymous (unless an individual wishes to give us their contact information) and will not impact any services one may receive or wish to receive in the future.
